Services

Free Consultation — $0 / Initial 15-minute call

A brief call to talk through what you are looking for, answer initial questions, and see whether Forget-Me-Not feels like a good fit.

This is not an evaluation, therapy session, or individualized clinical guidance appointment. All new clients begin here before scheduling another service.

Individual Therapy — $115 / 40-minute session

Individual therapy is personalized based on the person, their goals, and the kind of support that makes sense for them.

Sessions may include direct therapy, caregiver participation, communication partner training, education, practical strategies, advocacy, or real-life problem-solving.

Communication Support Session — $85 / 30-minute session

A focused session for adult clients, parents, caregivers, or communication partners seeking individualized guidance without beginning a full evaluation or ongoing therapy plan.

Sessions may include education, practical strategies, help understanding a communication profile or existing report, communication partner training, or support applying recommendations in everyday life.

This service may be recommended after the free consultation when a focused session is more appropriate than an evaluation or ongoing therapy.

Speech-Language Evaluations — $325 – $425

Evaluation fees vary based on the scope of the assessment. Evaluations are typically completed in one 60–90-minute appointment, although the format may vary depending on the client’s needs.

Every evaluation includes a case history, assessment, clinical interpretation, caregiver or communication partner input when appropriate, a written report, and recommendations for next steps. Recommendations and goals are informed by the client’s needs, priorities, communication environment, and lived experience.

Depending on the clinical question, an evaluation may also include formal testing, informal observation, language sampling, record review, AAC or communication-system review, multilingual considerations, Deaf and hard-of-hearing communication considerations, or input across home, school, work, or community settings.

Evaluations toward the lower end of the range are generally more focused, with one primary question, one main area of concern, and a more straightforward written report.

Evaluations toward the higher end of the range generally require a broader look across communication areas, languages, communication methods, records, settings, or support needs.

The evaluation fee, recommended format, and expected appointment length will be discussed before scheduling.
